WAIVER & RELEASE

Buyers and Student(s) agree that Student(s) engage in physical exercise, the use of equipment, and the use of Champions Creed training and instruction facility, which can be dangerous to the Student(s) and could cause injury to Student(s).  Student(s) is voluntarily participating in these activities and Buyer and Students(s) assume all risks of injury to Student.  Buyer and Student(s) hereby waive and release any claim or right to sue Champions Creed, employees or agents for injury to Student(s).  Buyer and /or Student(s) have carefully read this waiver and release and fully understand it is a release of all liabilities and damages to Champions Creed because of any injury that may occur.  Champions Creed will make no evaluation or recommendation on whether Student(s) or guests are physically fit for any exercise activities.  It is always advisable to consult your physician before undertaking a physical exercise program, particularly martial arts activities. 

LOSS/DAMAGE/THEFT OF PROPERTY  

Champions Creed does not assume any responsibility for the loss, damage or theft of any property belonging to the Student and Student agrees that the facility and its personnel are not responsible for or liable for any such property even if its loss, damage, or theft occurs on or about the facility.

CONSENT TO USE PHOTOGRAPHIC, VIDEO RECORDINGS:  

I consent that photographs may be taken and video recordings may be made of Buyers and/or Student(s) by Champions Creed, its affiliated and related companies and each of their representative directors, officers, owners, employees, representatives and authorized agents during the Student(s) participation in Champions Creed activities.  Such photographs and video recordings may be used by Champions Creed for current and future marketing activities and promotional materials.  I AUTHORIZE Champions Creed to use, reproduce, edit, alter, publish, transmit, distribute, broadcast and display photographs and video recordings that contain my child’s image, with or without the name associated with them, in any current or future promotional materials, in both print and electronic format, including, but not limited to:  publications, video multimedia productions, CD, DVD, displays, posters, brochures, advertisements, newsletters, media coverage, souvenirs, and on its website or social media without further notice to me or without my approval of the finished photographs, videos or audio recordings.

ACKNOWLEDGEMENT

I certify the information provided to be true and in consideration of the granting of this registration to me with the privileges incident thereto, and by registering I have become subject to the rules, regulations and decisions of Champion’s Creed,and I agree to abide by such rules, regulations and decisions of Champion’s Creed. Further, the information to be provided is required by Champion’s Creed to provide services and programs on behalf of the registrant and Champion’s Creed. Champion’s Creed will treat this personal information with the utmost respect and in accordance with the Champion’s Creed Privacy Policy at all times.
